OUR TEAM

Medical Director

Certifies the attending physician's prognosis, leads the hospice team in developing a care plan, and consults the attending physician.

Registered Nurses

Licensed nurses work with the hospice care team to provide pain and symptom management and comfort measures to your loved ones.

Home Nurse Making Bed

Certified Home Health Aid

Supports your loved ones by providing assistance with personal care and with daily living activities such as ambulation, transfers, toileting, bathing, dressing, and eating. 

Child Counselor

Spirit Counselor

Our non-denominational chaplain makes initial contact with each patient and their families to asses, and give spiritual counsel. Assist with funeral and memorial preperations.

Social

Social Worker

Evaluates the patient's and families' psychosocial needs and concerns, will provide emotional support and active listening, and will assist with advanced directives and final arrangements. 

Volunteering Group

Trained Volunteers

Trained volunteers offer compassionate support, companionship, and assistance to patients and families, including emotional support, daily living assistance, companionship, errands, and team collaboration.
